Writing a plan of correction that gets accepted the first time

Every acceptable plan answers five questions: how the affected residents were corrected, how others potentially affected were identified, what systemic change prevents recurrence, how the change will be monitored, and by what date.

Name a title, not a person, as responsible. Staff leave; the position remains accountable.

Avoid the word 'reeducated' as your only systemic action. Training alone rarely reads as a system change.

Monitoring must have a frequency, a sample size and an endpoint that routes back to QAPI.
